Northable - NASC - Kaikohe
NASC is a gateway to access disability support services for people under 65 years. The Ministry of Health contracts NorthAble to provide this service which is free to access. The first step is an assessment of a person's needs and identifies future goals and outcomes requird for the person to remain safely in the community. The assessor meets with the person and requested support people. The Needs Assessment report is prepared with and signed off by the person being assessed.
The Service Coordinator then develops a support plan with the person and authorises a package of disability services to meet their needs, taking into account resource constraints. Examples of disability support services could include household and personal support and assistance, community support, carer support subsidy, respite, supported living and residential support services.
A package of services may also include referrals to other agencies and professionals – for example, for assessment of equipment needs, rehabilitation support, day activity programmes, and personal therapies.
