ABI Rehabilitation Services

Cavit ABI Rehabilitation is contracted by ACC and MOH to provide specialist post-acute intensive rehabilitation to people with an acquired brain injury, including concussion and stroke. Access Ability Limited - Auckland

The budget is a set amount of funding made available each year for eligible people living in the Auckland (from Meremere north) and Northland regions. accessable manages applications for people seeking adaptive equipment, housing modifications, and vehicle purchase and/or modification. It also provides an equipment repair and maintenance service.

accessable manages a website which allows people to view equipment in both written and pictorial versions. The accessable store located in Auckland, provides a storage facility for bulk purchased items of equipment awaiting distribution and a central base for equipment collected from people who no longer have a requirement for the item.

accessable is also contracted to provide the services of a Housing Advisor. This person is available to assist specialised assessors with technical input, support and advice regarding housing modifications and to work with contractors to provide cost effective housing alteration solutions. Professional Advisors are employed by accessable for the purpose of reviewing the applications received from the specialised assessors to ensure recommendations made for EMS funding meet the eligibility and priority criteria.

Auckland DHB - NASC

As part of its Services to Older People, Auckland DHB has a Needs Assessment and Service Coordination (NASC) team that supports elderly people to access services that will assist them to stay in their own home. The service is available to people aged 65 years and over who live in the central Auckland area. This area covers the Western Bays, Hobson, Eastern Bays, Balmoral, Avondale/Roskill, Penrose and Hauraki Gulf Islands area of Auckland City. To access services such as home help or support with personal cares a person must have a needs assessment to determine their eligibility for services and the level of support required. This will usually involve a needs assessor visiting a person in their home to identify what support is required. A person can have support people present. Auckland DHB - Rehab Plus

Rehab Plus provides rehabilitation services to adults aged 16-64 years living in the Auckland/North Health region. Housing New Zealand is also the principal advisor to the Government on housing.

For people with physical disabilities who require modified housing Housing New Zealand has an advisory service called 'Suitable Homes'. To access this service the person needs to live in New Zealand, have a long term disability and require structural modifications to the home. The person does not need to be a Housing New Zealand tenant - it can be their own home or a rented home. The service also helps people with physical disabilities find homes that suit their needs. If modifications are required or the person needs to move to a new home an occupational therapist will assess the persons needs in relation to housing. If the person is eligible for funding through MOH or ACC the occupational therapist will make an application for housing modifications. The occupational therapist will also provide information on other sources of funding if the person is unable to afford the cost of modifications.

Northable - NASC - Kaikohe

NASC is a gateway to access disability support services for people under 65 years. The Ministry of Health contracts NorthAble to provide this service which is free to access. The first step is an assessment of a person's needs and identifies future goals and outcomes requird for the person to remain safely in the community. The assessor meets with the person and requested support people. The Needs Assessment report is prepared with and signed off by the person being assessed.

The Service Coordinator then develops a support plan with the person and authorises a package of disability services to meet their needs, taking into account resource constraints. Examples of disability support services could include household and personal support and assistance, community support, carer support subsidy, respite, supported living and residential support services.

A package of services may also include referrals to other agencies and professionals – for example, for assessment of equipment needs, rehabilitation support, day activity programmes, and personal therapies.

NZCare Group

NZCare Group, a division of Healthcare New Zealand Ltd, is committed to giving people of all ages and abilities the opportunity to be supported with dignity and respect. To help achieve this we provide a variety of supported living, residential and respite services to people with intellectual and/or physical disabilities. We also provide services such as Kaupapa Māori services, specialist physical disability services and transdisciplinary services.

For respite services in the Auckland area priority is given to people aged between 16 and 30 years with autism/intellectual disability or a physical disability. Out of home respite services for people with intellectual disability and Autism are available in Henderson, New Lynn and Weymouth, and a further respite service in Flat Bush will be opened in late June 2007.

Ranfurly Care - Epsom

Ranfurly Care Society provides a range of services for people with an intellectual disability. The Activity Centre provides a service Monday to Friday 8.30 to 3.30 daily.

We are a parent-based Society who recognise the diverse level of needs for each individual student by providing a comprehensive range of activities suited to their personal requirements. Each student has a daily programme to develop physical and recreational pursuits, and living skills. The Ranfurly Residential Homes provides 24 hour support for permanent residents with intellectual disabilities over the age of 16 years. We link people to community supports. The centre specializes in helping people to get their questions answered so that they can made informed decisions to improve their quality of life. Visionary Living

Supported Independent Living (SIL) services work with you to assist you to achieve the best possible level of independence in all aspects of your life. The SIL model gives you a range of options that are comprehensive and flexible and assist you to lead a sustainable and independent lifestyle in the community.

SIL services in Auckland are delivered by four contracted SIL providers. Referral to the services is made through the Needs Assessment and Service Coordination agency, Taikura Trust (Ph 09 278 6314).

Our Visionary Living service is managed by Latoa Halatau, who has over 25 years of social work experience. We work with people of all ethnicities and disabilities who are aged between 16 and 65 years who live in the greater Auckland area. Waitemata DHB - NASC

Waitemata DHB NASC aims to support a person to stay as independent as possible in their own home and community. The NASC service will work with a person to assess, plan and coordinate their support services. These include support with personal cares, household management, support for carers to take time out, day programmes and access to rest homes and private hospitals. NASC can also advise a person on services which are income or asset-tested and have entry criteria. The NASC service is available to people aged 65 years and over who live in the Waitemata District Health Board (North Shore, Waitakere and Rodney) area.
