Accessable

accessable (Environmental Health Management Services Limited) is based in Auckland and is the contracted provider responsible for administering the Equipment and Modification Services (EMS) budget on behalf of the Ministry of Health, Disability Services.

The budget is a set amount of funding made available each year for eligible people living in the Auckland (from Meremere north) and Northland regions. accessable manages applications for people seeking adaptive equipment, housing modifications, and vehicle purchase and/or modification. It also provides an equipment repair and maintenance service.

accessable manages a website which allows people to view equipment in both written and pictorial versions. The accessable store located in Auckland, provides a storage facility for bulk purchased items of equipment awaiting distribution and a central base for equipment collected from people who no longer have a requirement for the item.

accessable is also contracted to provide the services of a Housing Advisor. This person is available to assist specialised assessors with technical input, support and advice regarding housing modifications and to work with contractors to provide cost effective housing alteration solutions. Professional Advisors are employed by accessable for the purpose of reviewing the applications received from the specialised assessors to ensure recommendations made for EMS funding meet the eligibility and priority criteria.

accessable is also contracted by ACC to provide equipment and a repair service to ACC clients living in the Waikato, Auckland and Northland regions.

As part of its Services to Older People, Auckland DHB has a Needs Assessment and Service Coordination (NASC) team that supports elderly people to access services that will assist them to stay in their own home. The service is available to people aged 65 years and over who live in the central Auckland area. To access services such as home help or support with personal cares a person must have a needs assessment to determine their eligibility for services and the level of support required. Northland DHB - NASC - Kaitaia

The NASC service undertakes an assessment of people aged 65 and over who have an age-related disability, to determine their current and ongoing needs and to establish the most appropriate long term supports. These supports may range from informal supports such as family and friends, or formal supports such as cleaning service and personal cares or residential care. Access to subsidised supports must be via a NASC agency.

Anyone can refer to the NASC service but client consent is required.

Referrals can be made over the phone or via our referral form and returned via standard mail, email or fax. See contact details below.

For those living outside of the toll free calling area of Whangarei, we have an 0800 number to enable ease of access to our service.

The NASC service is part of Health of Older People. Health of Older People also includes the Health Monitoring for Older People team and Psychiatry for Older People team. NASC can be located at the following Northland district hospitals. The area they cover is also identified.

Whangarei Base Hospital which covers Brynderwyn, Hikurangi, Mangawhai, Ruakaka, Titoki and Waipu.

Dargaville Hospital which covers Awakino, Maungaturoto and Ruawai. Bay of Islands Hospital located in Kawakawa covers Kawakawa, Kerikeri, Kaikohe, Paihia and Russell.

Ranfurly Care - Epsom

Ranfurly Care Society provides a range of services for people with an intellectual disability. The Activity Centre provides a service Monday to Friday 8.30 to 3.30 daily.

We are a parent-based Society who recognise the diverse level of needs for each individual student by providing a comprehensive range of activities suited to their personal requirements. The Ranfurly Residential Homes provides 24 hour support for permanent residents with intellectual disabilities over the age of 16 years. Taikura Trust

TalkLink - Whangarei

TalkLink is a charitable trust that provides assistive communication technology services for people who have communication difficulties throughout the North Island. TalkLink Auckland covers the Northland, Waitemata, Auckland, Counties Manukau, Waikato, Lakes and Bay of Plenty District Health Board areas.

TalkLink services include assessments, equipment trials as part of the assessment process, completion of funding applications for assistive technology and training and support in the use of communication strategies and technology. Referral forms for assessments can be requested by phone, fax or email. A referral form can also be downloaded from the TalkLink website.

TalkLink Auckland is primarily staffed by speech language therapists and occupational therapists and has a therapist based in Whangarei. TalkLink has a contract with the Ministry of Health which means that assessment services are often provided at no cost.

Te Whanau O Waipareira Trust (Wai Health)

Wai Health has a multidisciplinary team of 106 full-time staff, including community health workers, well child nurses, public health specialists, dietician, doctors, practice nurses, diabetes nurse specialists, podiatrist, mental health workers, drug and alcohol counsellors, psychologist, ACC support workers. The Trust also contracts dental, optometry and physiotherapy services. We hope this year to provide pharmacy services and midwifery clinics. Wai Health is funded by Waitemata District Health Board and the Ministry of Health, with the exception of our ACC contract. This means a close working relationship with Waitemata District Health Board and the Ministry of Health is essential to building our capacity and capability to deliver quality services to our whanau.

Waitemata DHB - NASC

Waitemata DHB NASC aims to support a person to stay as independent as possible in their own home and community. The NASC service will work with a person to assess, plan and coordinate their support services. These include support with personal cares, household management, support for carers to take time out, day programmes and access to rest homes and private hospitals. NASC can also advise a person on services which are income or asset-tested and have entry criteria. The NASC service is available to people aged 65 years and over who live in the Waitemata District Health Board (North Shore, Waitakere and Rodney) area.

Wilson Centre

The Wilson Centre for children is a child disability service of Waitemata DHB. The Wilson Centre provides family-centred rehabilitation services for children aged 0-16 years (or while still at school) with neurological or orthopaedic problems. Rehabilitation programmes are tailored to meet the individual needs of the child or young person, and may include inpatient, day patient or outpatient options. Rehabilitation programmes may involve nurses, occupational therapists, speech language therapists, play specialists, social workers, clinical neuropsychologists, and a Maori Liaison Advisor. Facilities include gardens, purpose built playground, hydrotherapy pool, rehabilitation school classroom, and parent accommodation.

The Wilson Centre also offers free respite services for families of children aged 0-16 years with high support needs. Children are cared for in a safe, professional and fun environment while families take time out to recharge their batteries. Planned stays may include day stays, overnight weekdays, weekends and school holidays. 24 hour care is provided by nurses and healthcare assistants.
