Auckland DHB - Rehab Plus
Rehab Plus provides rehabilitation services to adults aged 16-64 years living in the Auckland/North Health region. Services are available to those who have been admitted to hospital following an illness, accident or injury (such as stroke, traumatic brain injury, other neurological condition, amputation, multiple trauma, complex orthopaedic/medical problems), who would benefit from short term rehabilitation and who meet other specified criteria.
People attending Rehab Plus are helped to return to their maximum potential using individual programmes to achieve their goals. Services available include inpatient rehabilitation and nursing care, outpatient rehabilitation, and wheelchair/seating assessment.

Elder Family Matters has assisted older people in the wider Wellington region (including Hutt Valley, Porirua Basin, Kapiti Coast, and Wairarapa) for 10 years so they can choose to continue living in the comfort and familiarity of their own homes. Elder Family Matters do this by: •assisting the elderly and disabled with the tasks of daily living: shopping, housework and cleaning, personal cares, laundry, meals, transport, social activities, appointments, home maintenance; •one-on-one support / companionship / outings for people in residential care; •respite care to enable primary carers to have a break. Full range of Carers including highly experienced carers, registered nurses, enrolled nurses, occupational therapists, and physiotherapists. Northland DHB - Wheelchair/Seating Service Northland
The Wheelchair/Seating Service Northland is a service of the Northland District Health Board and is located in a separate building at Whangarei Base Hospital. It is contracted by the Ministry of Health, Disability Services to provide Level 2 and 3 wheelchair and seating assessments to people eligible to receive Environmental Support Services funding for equipment.
The service is provided to people living in Northland from north of Wellsford to Cape Reinga and can be accessed using a referral form provided by the service. Upon receipt of a referral an acknowledgement letter and information pack is sent out and people are prioritised onto a waiting list. Wheelchair Seating Therapists perform the specialised assessments either in a person's home or at the Wheelchair/Seating Service in Whangarei.
Wheelchair/Seating Service Northland is also contracted by accessable to provide a repairs and maintenance service. This service can be contacted on 09 430 4101 ext 7157.

A group for mobility scooter users, coordinated by Horizons Regional Council Roadsafe Coordinator Whanganui. Meets every third Wednesday of the month, excluding January, at The Brick House for lunch at 11.30am. Sometimes have a guest speaker. Have a Scooter Skills day once a year and members can attend a scooter day in Palmerston North once a year to meet up with other scooter groups from around the region. Taranaki Disabilities Information Centre
We link people to community supports. The centre specializes in helping people to get their questions answered so that they can made informed decisions to improve their quality of life. This is a free information and advisory service available to everyone; people with a disability, family and whanau, support workers and support organisations. We have equipment available for purchase and hire. Secondhand equipment can also be sold through us. If there is something that you need but we don't stock it we will try to source it. Barrier Free assessment service is available, information on accessible homes. We have information on accessible motels, hotels and activities. We can put you in contact with over 50 support groups and link you to their support network. We have an accessible meeting room and kitchen for hire. This is set up with digital projector and a Front Row 2 Go system hearing system. The Centre is open 9-5 Monday to Friday. Creative Space is an accessible art group that operates on Fridays from 10-3. Please call the Centre if you would like to attend.

We offer a free Disability Information Service which is open to anybody and to any organisation, and a free Supported Employment Service which people may self refer to, or be referred to. This includes Transition support for students transitioning from school. We also provide an Equipment Loan Service and Sales of Aids and Equipment. We cover Whanganui, Rangitikei and Ruapehu districts and South Taranaki.
