Auckland DHB - NASC

As part of its Services to Older People, Auckland DHB has a Needs Assessment and Service Coordination (NASC) team that supports elderly people to access services that will assist them to stay in their own home. The service is available to people aged 65 years and over who live in the central Auckland area. This area covers the Western Bays, Hobson, Eastern Bays, Balmoral, Avondale/Roskill, Penrose and Hauraki Gulf Islands area of Auckland City. To access services such as home help or support with personal cares a person must have a needs assessment to determine their eligibility for services and the level of support required. This will usually involve a needs assessor visiting a person in their home to identify what support is required. A person can have support people present. Auckland DHB - Rehab Plus

Rehab Plus provides rehabilitation services to adults aged 16-64 years living in the Auckland/North Health region. Services are available to those who have been admitted to hospital following an illness, accident or injury (such as stroke, traumatic brain injury, other neurological condition, amputation, multiple trauma, complex orthopaedic/medical problems), who would benefit from short term rehabilitation and who meet other specified criteria.

People attending Rehab Plus are helped to return to their maximum potential using individual programmes to achieve their goals. Services available include inpatient rehabilitation and nursing care, outpatient rehabilitation, and wheelchair/seating assessment.

Autism New Zealand Inc - Auckland

Autism New Zealand provides support, resources and information on autism spectrum disorders including Asperger syndrome and related disorders.

Autism New Zealand's key activities include raising awareness of autism spectrum disorders, providing support, information and advice. This includes gathering and distributing the latest information and research relating to autism spectrum and related disorders. Autism New Zealand does this by maintaining contact with overseas associations. They will arrange seminars and conferences and provide training courses for parents and professionals. They will also liaise with other agencies on behalf of individuals, parents and other family.

Autism New Zealand resources and maintains a library service and information can be accessed by contacting their Information Officer. This person is based at their national office in Christchurch. Brain injuries, also sometimes referred to as head injuries fall into two groups. An Acquired Brain Injury (ABI) can occur as a result of strokes, tumours and other medical problems including drug and alcohol abuse. A Traumatic Brain Injury (TBI) occurs when damage is caused to the brain by an external force such as in a car accident, assault or sporting accident. TBI's are commonly covered by ACC. The most common TBI is concussion. BIANZ employs Liaison Officers and Education Advisors.

The New Zealand Audiological Society (NZAS) represent a number of audiologists throughout New Zealand. Audiologists, who are members of the NZAS are the specialised assessors approved to assess and prescribe equipment for hearing. Therefore they are the professional group that people with hearing impairment must access to be assessed for ESS funded support for hearing aids. NZAS Audiologists are also approved service providers for financial assistance through ACC, War Pensions, Government subsidies and Work and Income. The majority of audiologists work in the private sector and as such there is typically a cost associated with an assessment.

NZCare Group, a division of Healthcare New Zealand Ltd, is committed to giving people of all ages and abilities the opportunity to be supported with dignity and respect. To help achieve this we provide a variety of supported living, residential and respite services to people with intellectual and/or physical disabilities. We also provide services such as Kaupapa Māori services, specialist physical disability services and transdisciplinary services.

For respite services in the Auckland area priority is given to people aged between 16 and 30 years with autism/intellectual disability or a physical disability. Out of home respite services for people with intellectual disability and Autism are available in Henderson, New Lynn and Weymouth, and a further respite service in Flat Bush will be opened in late June 2007.

Referrals for Auckland respite services are achieved by contacting the NASC agency Taikura Trust who will complete a needs assessment, allocate respite days and then place a referral to NZCare. More information about NZCare and its services can be found on the website listed below.

The Ranfurly Residential Homes provides 24 hour support for permanent residents with intellectual disabilities over the age of 16 years. Supported Independent Living (SIL) services work with you to assist you to achieve the best possible level of independence in all aspects of your life. The SIL model gives you a range of options that are comprehensive and flexible and assist you to lead a sustainable and independent lifestyle in the community.

SIL services in Auckland are delivered by four contracted SIL providers. Referral to the services is made through the Needs Assessment and Service Coordination agency, Taikura Trust (Ph 09 278 6314).

Waitemata DHB NASC aims to support a person to stay as independent as possible in their own home and community. NASC can also advise a person on services which are income or asset-tested and have entry criteria. Wilson Centre

The Wilson Centre for children is a child disability service of Waitemata DHB. The Wilson Centre provides family-centred rehabilitation services for children aged 0-16 years (or while still at school) with neurological or orthopaedic problems. Rehabilitation programmes are tailored to meet the individual needs of the child or young person, and may include inpatient, day patient or outpatient options. Rehabilitation programmes may involve nurses, occupational therapists, speech language therapists, play specialists, social workers, clinical neuropsychologists, and a Maori Liaison Advisor. Facilities include gardens, purpose built playground, hydrotherapy pool, rehabilitation school classroom, and parent accommodation.

The Wilson Centre also offers free respite services for families of children aged 0-16 years with high support needs. Children are cared for in a safe, professional and fun environment while families take time out to recharge their batteries. Planned stays may include day stays, overnight weekdays, weekends and school holidays. 24 hour care is provided by nurses and healthcare assistants.
