The purpose or thinking of the time was to support disabled people and their families/whanau to pursue meaningful lives and to recognise that the NASC process would have a far-reaching impact on their ability to do that. Both Lorna and Jan were working within provider organisations at the time, CCS Bay of Plenty, and Community Living Trust. A nominal capital was raised, together with the donation of personal resources, including two foundation Board Members, Kaye Taylor and Peter McRae. During this time, the Access Ability Board was very much a working Board, with the ongoing development of the company and the preparation of proposals being undertaken by Jan and Lorna, together with financial and accounting input from Kaye. Community Living Trust also allowed Access Ability to contract a portion of Jan’s time to act as Managing Director during these formative years.

accessable (Environmental Health Management Services Limited) is based in Auckland and is the contracted provider responsible for administering the Equipment and Modification Services (EMS) budget on behalf of the Ministry of Health, Disability Services.

The budget is a set amount of funding made available each year for eligible people living in the Auckland (from Meremere north) and Northland regions. accessable manages applications for people seeking adaptive equipment, housing modifications, and vehicle purchase and/or modification. It also provides an equipment repair and maintenance service.

accessable manages a website which allows people to view equipment in both written and pictorial versions. The accessable store located in Auckland, provides a storage facility for bulk purchased items of equipment awaiting distribution and a central base for equipment collected from people who no longer have a requirement for the item.

accessable is also contracted to provide the services of a Housing Advisor. This person is available to assist specialised assessors with technical input, support and advice regarding housing modifications and to work with contractors to provide cost effective housing alteration solutions. Professional Advisors are employed by accessable for the purpose of reviewing the applications received from the specialised assessors to ensure recommendations made for EMS funding meet the eligibility and priority criteria.

accessable is also contracted by ACC to provide equipment and a repair service to ACC clients living in the Waikato, Auckland and Northland regions.

Housing New Zealand is also the principal advisor to the Government on housing.

For people with physical disabilities who require modified housing Housing New Zealand has an advisory service called 'Suitable Homes'. To access this service the person needs to live in New Zealand, have a long term disability and require structural modifications to the home. The person does not need to be a Housing New Zealand tenant - it can be their own home or a rented home. The service also helps people with physical disabilities find homes that suit their needs. If modifications are required or the person needs to move to a new home an occupational therapist will assess the persons needs in relation to housing. If the person is eligible for funding through MOH or ACC the occupational therapist will make an application for housing modifications. The occupational therapist will also provide information on other sources of funding if the person is unable to afford the cost of modifications.

Northable - NASC - Kaikohe

NASC is a gateway to access disability support services for people under 65 years. The Ministry of Health contracts NorthAble to provide this service which is free to access. The first step is an assessment of a person's needs and identifies future goals and outcomes requird for the person to remain safely in the community. The assessor meets with the person and requested support people. The Needs Assessment report is prepared with and signed off by the person being assessed.

The Service Coordinator then develops a support plan with the person and authorises a package of disability services to meet their needs, taking into account resource constraints. Examples of disability support services could include household and personal support and assistance, community support, carer support subsidy, respite, supported living and residential support services.

A package of services may also include referrals to other agencies and professionals – for example, for assessment of equipment needs, rehabilitation support, day activity programmes, and personal therapies.

Northland DHB - Community Assessment Rehabilitation Service (CARS)

The Community Rehabilitation Service has four interdisciplinary teams based in Whangarei, Dargaville, Bay of Islands and Kaitaia.

The teams provide assessment and rehabilitation for people living in the community, who have the potential to achieve goals and improve their level of function.

The teams accept referrals from health professionals. A person can be referred following stroke, TIA, subarachnoid haemorrhage, non-ACC brain injury, amputation or complex/lengthy hospital admissions. A small number of referrals are for people with neuro-degenerative conditions such as Multiple Sclerosis, Motor Neurone Disease and Parkinson’s Disease, where there is scope for short-term improvement.

Our aim is to assist people to participate in their community and to reach their full potential using a goal-oriented approach.

Northland DHB - NASC - Whangarei

The NASC service undertakes an assessment of people aged 65 and over who have an age-related disability, to determine their current and ongoing needs and to establish the most appropriate long term supports. These supports may range from informal supports such as family and friends, or formal supports such as cleaning service and personal cares or residential care. Access to subsidised supports must be via a NASC agency.

Anyone can refer to the NASC service but client consent is required.

Referrals can be made over the phone or via our referral form and returned via standard mail, email or fax. See contact details below.

For those living outside of the toll free calling area of Whangarei, we have an 0800 number to enable ease of access to our service.

The NASC service is part of Health of Older People. Health of Older People also includes the Health Monitoring for Older People team and Psychiatry for Older People team. NASC can be located at the following Northland district hospitals. The area they cover is also identified.

Northland DHB - Wheelchair/Seating Service Northland

The Wheelchair/Seating Service Northland is a service of the Northland District Health Board and is located in a separate building at Whangarei Base Hospital. It is contracted by the Ministry of Health, Disability Services to provide Level 2 and 3 wheelchair and seating assessments to people eligible to receive Environmental Support Services funding for equipment.

The service is provided to people living in Northland from north of Wellsford to Cape Reinga and can be accessed using a referral form provided by the service. Upon receipt of a referral an acknowledgement letter and information pack is sent out and people are prioritised onto a waiting list. Wheelchair Seating Therapists perform the specialised assessments either in a person's home or at the Wheelchair/Seating Service in Whangarei.

Wheelchair/Seating Service Northland is also contracted by accessable to provide a repairs and maintenance service. This service can be contacted on 09 430 4101 ext 7157.

NZCare Group

NZCare Group, a division of Healthcare New Zealand Ltd, is committed to giving people of all ages and abilities the opportunity to be supported with dignity and respect. To help achieve this we provide a variety of supported living, residential and respite services to people with intellectual and/or physical disabilities. We also provide services such as Kaupapa Māori services, specialist physical disability services and transdisciplinary services.

For respite services in the Auckland area priority is given to people aged between 16 and 30 years with autism/intellectual disability or a physical disability. Out of home respite services for people with intellectual disability and Autism are available in Henderson, New Lynn and Weymouth, and a further respite service in Flat Bush will be opened in late June 2007.

Referrals for Auckland respite services are achieved by contacting the NASC agency Taikura Trust who will complete a needs assessment, allocate respite days and then place a referral to NZCare. More information about NZCare and its services can be found on the website listed below.

Taikura Trust

Taikura Trust is the agency contracted by the Ministry of Health, Disability Services to provide a Needs Assessment and Service Coordination (NASC) service to people aged under 65 years who live in the greater Auckland region.

Taikura Trust assesses a person's eligibility to access disability support services and work with all the services available in the region. Their services cover the area between Wellsford in the north to Mercer in the south.
