Hauora Hokianga Integrated PHO provides a range of primary health services to the predominantly Maori population residing in the relatively isolated and rural Hokianga area. Work and Life skills are all part of the week day activities at Kauilands. The core aim of this programme, in line with the Ministry of Social Developement's directive, is to encourage each resident to achieve where possible, their personal goals and aspirations for a normal life. Barrier Free assessment service is available, information on accessible homes. We have information on accessible motels, hotels and activities. We can put you in contact with over 50 support groups and link you to their support network. We have an accessible meeting room and kitchen for hire. This is set up with digital projector and a Front Row 2 Go system hearing system. The Centre is open 9-5 Monday to Friday. Supported Independent Living (SIL) services work with you to assist you to achieve the best possible level of independence in all aspects of your life. The SIL model gives you a range of options that are comprehensive and flexible and assist you to lead a sustainable and independent lifestyle in the community.
SIL services in Auckland are delivered by four contracted SIL providers. Referral to the services is made through the Needs Assessment and Service Coordination agency, Taikura Trust (Ph 09 278 6314).
Our Visionary Living service is managed by Latoa Halatau, who has over 25 years of social work experience. We work with people of all ethnicities and disabilities who are aged between 16 and 65 years who live in the greater Auckland area. Some areas of assistance include finding appropriate accommodation, managing your home and financial affairs, travelling safely, training, and social and recreational activities.

Waitemata DHB NASC aims to support a person to stay as independent as possible in their own home and community. The NASC service will work with a person to assess, plan and coordinate their support services. These include support with personal cares, household management, support for carers to take time out, day programmes and access to rest homes and private hospitals. NASC can also advise a person on services which are income or asset-tested and have entry criteria. Wilson Centre
The Wilson Centre for children is a child disability service of Waitemata DHB. The Wilson Centre provides family-centred rehabilitation services for children aged 0-16 years (or while still at school) with neurological or orthopaedic problems. Rehabilitation programmes are tailored to meet the individual needs of the child or young person, and may include inpatient, day patient or outpatient options. Rehabilitation programmes may involve nurses, occupational therapists, speech language therapists, play specialists, social workers, clinical neuropsychologists, and a Maori Liaison Advisor. Facilities include gardens, purpose built playground, hydrotherapy pool, rehabilitation school classroom, and parent accommodation.
The Wilson Centre also offers free respite services for families of children aged 0-16 years with high support needs. Children are cared for in a safe, professional and fun environment while families take time out to recharge their batteries. Planned stays may include day stays, overnight weekdays, weekends and school holidays. 24 hour care is provided by nurses and healthcare assistants.
